RealCare is registered with Department of Health and NDIA to offer Home Care Services in specific registration groups. We manage our size and geographical spread to enable personalised touch to our services.
What experience do you have in delivering Home Care?
RealCare has been in operation in 2006. Our Care support workers are experienced and able to handle services that we commit to deliver.
What geographical locations do you cover?
RealCare covers the Metropolitan suburbs of Western Melbourne. However, service recipients in the adjoining areas may be considered. Please contact us for further details.
What level of flexibility and choice do you allow?
RealCare is committed to Consumer Directed Care as required in the Aged Care Standards. Every aspect of care will be agreed with you at the start and documented in Care Service Agreement.
Do you work with people of all backgrounds?
RealCare is multicultural and welcomes people from Culture and Language diverse backgroup.
Some of our staff are Bilingual and interpreters can be arranged if necessary,
Can you meet with me in person?
Our Care Services Manager or Office manager are available to meet to discuss your requirements.
Can I speak directly with my Case Manager?
Yes, you will be provided with the Care Services Manager contacts. If the Care Services Manager is unavailable at any given time during business hours, an on-call case manager will always be on hand to assist you. Your Case Manager will also visit you at mutually agreed upon times for reviews and you may request additional visits if required.
Are you available after-hours?
Yes, all calls outside business hours are directed to our After Hours Assistance Service. Our office is closed weekends and public holidays; however we provide an after-hours service to ensure support is always available.
How are your fees charged?
We start by agreeing with you the services that are required. We charge hourly fee for service. The number of hours and types of services vary from Client to Client and this is stipulated in the Home Care Services Agreement.
How can you help me to maximize my funds?
Our Care Services and Finance Managers will work with you to ensure your funds are used to their full potential. Where practical we will provide services from our own worker or source the most competitivie quotes in the market. Furthermore, you will be kept abreast of your budget and expenses in a monthly statement or we can issue a statement at any time when requested.
If I already have a package with another service provider, can I transfer to Realcare?
Consumer Directed Care model that forms part of Aged Care Standards allows you transfer your package. It is important to understand your obligations with your current service. Also we will help you to make an informed decision by answering any questions you may have about our service.
Do my unspent funds transfer over with me if I change service providers?
Yes, you can select and redirect remaining unspent package funds to any service provider within Australia. If you decide to change over to RealCare, we will work closely with your previous provider to facilitate a smooth transition of you home care services and unspent funds to us.
Should you decide to change from RealCare to another service provider, there are minimum notice periods and other requirments that will be stipulated in the agreement.
