Privacy and Confidentiality
RealCare will require your personal and sensitive information in order to provide care. We have a strong commitment to maintaining the privacy and confidentiality of clients of our service in compliance with Australian Privacy Act 2001. We are required to report information to funding bodies as part of our contractual accountability about the services that we provide.
Information collected by RealCare
In your personal file we keep your name, contact details and other information, such as your care plan and your health and care needs. If required, your worker may write progress notes for each contact made with you. Other information collected includes:
- demographic information such as age, date of birth, address, ethnicity, contact information
- information we have collected from you during an assessment of your needs
- case notes indicating our contact with you or work on your behalf
- Timesheets and notes recorded by Support Workers when providing services
- information from other parties where you have consented to them to release such information.
The information will be collected from you directly and where not possible from people you have authorised or have been authorised through appropriate legal means.
Uses of Information Collected
The information we collect helps us to keep up-to-date details regarding your care needs and in doing so enables us to provide the best possible care for you. We also use the information to continually improve our service delivery.
Your duty of disclosure and maintaining currency of information
We rely on the information you provide to us in order to formulate a concise care plan that is tailored to your specific care needs. It is your right to not disclose personal information to RealCare, as long as it does not place or expose our either yourself or our staff to any risks as a result of your non-disclosure. The information we collect about you allows us to fulfill our duty of care. If we do not have sufficient information about your health care needs we may not be able to provide a service to you.
You have the right to request access to your personal information and ask for it to be amended if deemed necessary.
We also expect you to update your information as soon as changes occur so that quality of care and our duty of care is not compromised in any way. RealCare will not be held responsible for incidents due to lack of updated information.
RealCare does not disclose personal information to a third party else except-
- with the consent of the person
- to prevent a serious and imminent threat to a person’s health or safety.
- as required or authorised by law
- where reasonably necessary for the enforcement of criminal or revenue law.
Any person the information is provided to under any of these exceptions, may only use it for the purpose for which it was disclosed. RealCare will inform recipient third party of this condition but the conduct of the third party is beyond or control.
Your information is only available to RealCare personnel who are involved in your care provision.
Securing your information
RealCare is committed to protecting our consumer’s privacy and confidentiality to ensure that the dignity of individuals is upheld and respected at all times. Our service takes reasonable steps to protect the personal information we hold from misuse and loss, and from unauthorised access, modification or disclosure
All personal files are stored securely and all personal information is treated in the strictest of confidence.
It is your responsibility to keep secure any usernames or passwords provided to you and not share them with third parties who may gain unauthorised access to your electronic information or communication between us and you.
